Gargoyle Award Nomination Form

The deadline for nominations to be received at CDR is 5 PM, Monday, July 14th, 2003. You may nominate one person or organization per category. If you have questions about the awards or the nomination process, please call our office. The Awards Committee assumes that all nominees are aware that they have been nominated for this honor, so please be sure to notify the nominees before submitting their names. We do not accept faxed nominations or self-nominations. formmail.cgi

NOTE: The judges reserve the right to make an award in a category other than the one you select.
The Winged Gargoyle Award: This award recognizes a lifetime of work on behalf of the social interests of people with disabilities.
The Razzberry Gargoyle: This award rewards self advocacy or self-empowering action by a person with a disability.
The Gnawing Gargoyle Award: This award rewards the creation or improvement of a public policy benefiting people with disabilities.
The Crane Gargoyle Award: This award recognizes precedent-setting achievements in the application of rights, construction of access or the practice of accommodation for people with disabilities.
The Hooded Gargoyle Award (The Brian Hall Award): This award honors a professional person under 40 for services on behalf of people with disabilities.
The Dragon Gargoyle Award (The David Iris Award): This award recognizes contributions in child advocacy.
